Insurance and Fees

Dr. Toni currently accepts Medicare Part B and Aetna. Otherwise, Dr. Toni is considered an out of network provider (including United, Aetna, and Blue Cross Blue Shield Medicare Advantage plans). 

 

Services may be in-part or fully covered by your insurance provider. The full fee is expected at time of appointment, and Dr. Toni will provide a superbill for potential reimbursement directly to you. Reimbursement is dependent on in and out of newtwork benefits and plan details. Please call Dr. Toni at 954-271-3397 to discuss your insurance benefits, associated co-payments/co-insurance and deductibles, and potential need for approval by your primary care physician for services prior to the evaluation or the initial appointment. 

 

For patients who elect to not use their insurance, the cost of therapy sessions is $200. A sliding scale fee is available for those who qualify. Full fee or co-payment/co-insurance payment for individual therapy is expected at the time of service. Cash, check, and all major credit cards are the current accepted forms of payment.  For testing services, half of the fee is due at the initial appointment. This payment is non-refundable. The second half of payment is due prior to the feedback session. If utilizing insurance, please keep in mind testing services rendered may not be covered or may only be partially covered.

Cancelation Policy

24 hours advanced notice of cancelation is required. A cancelation fee of $70 is subject to be billed if you do not give at least 24 hours notice out of respect to my other clients who may want your appointment time.

The No Surprises Act

As of January 1, 2022 the No Surprises Act went into effect regarding healthcare services. This means if you are a cash pay patient or utilizing out of network benefits you are entitled to a Good Faith Estimate (GFE) from Dr. Toni by the date of your first appointment. 

The Good Faith Estimate shows the cost of services that are reasonably expected for your mental healthcare needs. The estimate is based on information known at the time the estimate is created.
